We facilitate the development of abilities and skills essential for participants to engage in typical community activities and functions, mirroring those chosen by the general population. This includes involvement in community education or training, retirement, and volunteer activities. Through our Community Connection program, we offer diverse opportunities to foster relationships and natural support networks within the community. We leverage the community as a dynamic learning environment to deliver identified services and support, enhancing participants’ integration and participation in community life such as:

Activities of choice that promote interaction with others.
Support from peers who share experiences, offering valuable modeling and advice.
Comprehensive support for integrated and meaningful learning opportunities, empowering individuals to make informed choices about their community involvement.
Helping individuals acquire, maintain, and enhance essential skills for community living, including self-help, socialization, self-advocacy, and adaptive skills.

These activities take place in diverse settings where participants engage with non-disabled individuals, excluding those directly providing services. Services encompass socialization, adaptive skills training, and personnel support during community outings. Necessary resources for participation and skill enhancement are provided, facilitating engagement and learning. Community Connections can occur in group settings or one-on-one, depending on the individual’s Service Plan, serving as instructional environments tailored to their needs and goals.
